New Patient exams typically involve getting personal and health history information, insurance information, a full mouth exam, a full mouth series of X-rays, diagnosis and treatment planning of any treatment necessary, and a cleaning.

In order the make this appointment run smoothly, patients can download and fax health history and personal information forms prior to their office visit. This allows our office time to obtain a fax copy of benefits from insurance companies. Patients are also encouraged to download and fax a sign release of records from your previous dentist. Often it avoids having to take X-rays if they are current.
